Clinical history

52-yo-male with PMH methamphetamine use presented with 1 month history of increasing DOE, orthopnea, and episodic atypical sharp chest pain. EKG revealed LVH with inferolateral T-wave inversions; troponin was indeterminate (making ACS less likely).
